4. The notice set out in sections 92 and 437 of the Act to seek the specific consent of a client in order to authorize a distributor to use information held by him for other purposes than the purposes for which it has been gathered shall be in accordance with the form set out in Schedule 3 - NOTICE OF SPECIFIC CONSENT.
